Did you know that Jewish Family & Children’s Service offers personalized in-home assessments to help older adults (age 55+) age in place safely with confidence and dignity? Conducted by Master’s level social workers, these proactive visits are designed to provide a thoughtful “getting to know you” baseline—so we can truly understand each individual’s needs, preferences, values, and goals before a crisis occurs.

Having an assessment in place ahead of time allows potential risks to be identified early, small concerns to be addressed before they become urgent, and thoughtful plans to be made without the pressure of an emergency. When a hospital stay, fall, or sudden change in health does happen, families are often forced to make difficult decisions quickly. A preemptive assessment helps reduce stress during those moments by ensuring that trusted supports, clear recommendations, and an action plan are already in place.

During the assessment, our team looks at the whole picture: daily routines, home safety, social supports, emotional well-being, and future planning needs. From there, we connect clients to a wide range of services—both in the home and throughout the community—including transportation, volunteer support, food programs, financial assistance, benefits guidance, and mental health resources.
